How strict should we be about enforcing approved work instructions under schedule pressure?

You should be very strict. In most regulated manufacturing environments, approved work instructions are part of the controlled method of execution. Schedule pressure may explain why people want to deviate, but it does not make uncontrolled deviation acceptable.

If an instruction is unclear, outdated, or impossible to follow as written, the right response is not to ignore it. The right response is to stop, escalate, and use the approved path for clarification, temporary deviation, concession, or revision, depending on your quality system and the nature of the issue.

What strict enforcement actually means

Strict enforcement does not mean pretending every instruction is perfect. It means:

  • operators are expected to follow the currently approved version
  • supervisors do not authorize verbal shortcuts outside the control process
  • known instruction defects are escalated quickly
  • temporary changes are documented, reviewed, and traceable
  • training and acknowledgment reflect the version actually in use

That is a different standard from rigidly pushing bad instructions. If the approved instruction is wrong, management owns the correction path.

Why schedule pressure is a weak basis for exception

Under schedule pressure, teams often assume the risk is limited to one job or one shift. In practice, the larger risks are cumulative:

  • loss of traceability for what was actually done
  • inconsistent execution between operators, shifts, or sites
  • rework, scrap, escapes, and delayed investigations
  • training drift, where informal practice replaces controlled process
  • evidence gaps during internal review, customer review, or audit

Those risks can exceed the short-term benefit of pushing work through. This is especially true where product acceptance, repair history, process qualification, or as-built records matter.

When some flexibility is reasonable

Flexibility is reasonable in how fast you resolve the problem, not in whether the control exists. The practical question is whether you have a usable mechanism for urgent cases. Mature organizations usually need some combination of:

  • rapid review and approval workflows
  • clearly defined authority limits for temporary deviations
  • visible version control at the point of use
  • immediate communication to affected operators and inspectors
  • follow-up change control to prevent repeat exceptions

If those mechanisms are slow or unclear, people will work around them. That is a process design problem, not proof that instruction control is unnecessary.

Brownfield reality

In mixed environments, enforcement is harder than policy suggests. Plants often have paper packets, shared drives, MES prompts, ERP travelers, legacy document control tools, and tribal knowledge all active at once. In that situation, strict enforcement depends less on writing another rule and more on whether the approved version is actually accessible, current, and connected to execution.

If operators must check one system for the traveler, another for drawings, and a third for the latest instruction revision, errors are predictable. Full replacement of every legacy system is often unrealistic in regulated, long-lifecycle operations because qualification burden, validation cost, downtime risk, and integration complexity are high. In many plants, the safer approach is phased control improvement: tighten document governance, improve point-of-use access, log acknowledgments, and connect evidence trails across existing systems.

Practical policy direction

A workable standard is usually:

  • no informal bypass of approved work instructions
  • yes to urgent escalation when the approved instruction blocks safe or conforming execution
  • yes to documented temporary authority where your quality system permits it
  • mandatory follow-up to correct the root cause in the instruction, training, or system design

If schedule pressure repeatedly drives instruction bypass, the underlying issue is rarely operator discipline alone. It is often one or more of these: poor instruction quality, slow approval cycles, weak change control, disconnected systems, or unrealistic production planning.

So the short answer is yes, be strict about enforcement. But also be equally strict about making the approved path fast enough, usable enough, and traceable enough that people can comply under real production conditions.
