CAPA root cause

A CAPA root cause is the underlying reason a quality issue occurred within a corrective and preventive action process.

A CAPA root cause is the underlying reason a quality issue, nonconformance, deviation, or recurring problem occurred within a Corrective and Preventive Action process. It identifies the cause that must be addressed for the CAPA to be meaningful, rather than only describing the visible defect or immediate symptom.

In manufacturing and quality systems, the CAPA root cause is commonly documented as part of a nonconformance, customer complaint, audit finding, process failure, or other quality event. It supports the selection of corrective actions, preventive actions, owners, due dates, and effectiveness checks in a quality management system, MES, or related workflow.

A CAPA root cause should not be confused with containment, correction, or disposition. Containment limits the impact of the issue, correction fixes the specific instance, and disposition determines what to do with affected material. The root cause explains why the issue happened so that an appropriate corrective or preventive action can be defined.
